AIDIS uses your name, email, and answers to assess your work and follow up about your assessment. Submissions are processed by FormSubmit for email delivery; its documentation states that submissions are retained for 30 days. AIDIS receives a copy by email. Please describe your work without including passwords, bank details, or private customer or employee information. This form does not sign you up for marketing.
